How to Enroll

Pension Plan

  1. USPS employees complete the EZ Retirement Plan Enrollment Form and select the option for the FRS Pension Plan. Submit the form directly to the Florida Retirement System (FRS) at the address or fax number listed on the form.
  2. A&P and Faculty employees complete the State University System Optional Retirement Program Retirement Plan Enrollment form to decline participation in the SUSORP. Also, complete the EZ Retirement Plan Enrollment Form and select the option for the FRS Pension Plan. Submit both forms to the Human Resources Benefits Office - by fax to (850) 645-9509.

DROP

  1. Complete the Drop Retirement Forms Packet.
  2. Complete the Drop Leave Election form.
  3. Once completed, turn the form(s) in to the Human Resources Benefits Office at A6200 University Center, or fax them to (850) 645-9509.

Investment Plan

  1. Employee will receive an information packet at their home approximately three months after date of hire.
  2. USPS employees complete the EZ Retirement Plan Enrollment Form and select the option for the FRS Investment Plan. Submit the form directly to the Florida Retirement System (FRS) at the address or fax number listed on the form.
  3. A&P and Faculty employees complete the State University System Optional Retirement Program Retirement Plan Enrollment form to decline participation in the SUSORP. Also, complete the EZ Retirement Plan Enrollment Form and select the option for the FRS Investment Plan. Submit both forms to the Human Resources Benefits Office - by fax to (850) 645-9509.

Optional Retirement Program (ORP)

  1. Contact the approved provider company of your choice to sign a contract with the company.
  2. Complete the State University System Optional Retirement Program Retirement Plan Enrollment Form and select the option for the ORP.
  3. Submit the form to the Human Resources Benefits Office - by fax to (850) 645-9509.

Bencor/FICA Alternative Program

  • Participation in this plan is mandatory for all eligible employees. Enrollment is automatic.
