Workers' Compensation

The purpose of the Workers' Compensation program is to coordinate and manage claims relating to work-related injuries sustained by University employees and volunteers. Some aspects of the claims process include reporting an injury, obtaining medical treatment, and returning the employee to work. The University policies and procedures can also provide guidance, especially in areas such as time & leave and payroll issues. 

Reporting Workplace Accidents

Please report any injury or illness to AmeriSys at 1-800-455-2079.

When an employee sustains a work-related injury or illness that requires emergency medical treatment, they should go directly to the hospital or call emergency medical services by dialing 911. The supervisor/designated department representative should immediately contact AmeriSys and provide the injured employee's name, where they are going for treatment, the employee's date of birth, mailing address, and social security number. AmeriSys will contact the medical facility to initiate medical case management and contact the supervisor once the emergency is under control. Additionally, the supervisor/designated department representative should complete an Accident Investigation Report and forward it to the Workers' Compensation Manager within 48 hours of the accident.

When an employee is injured and requires non-urgent or no medical treatment, the employee must immediately report the accident to the supervisor/designated department representative, who should immediately report the claim to AmeriSys. This will initiate medical treatment and create a record of the accident. Additionally, the Accident Investigation Report should be completed and forwarded to the Workers' Compensation Manager within 48 hours of the accident.

 

Should an injury occur resulting in the death of an employee, the supervisor/designated department representative should immediately report the death to the following: Senior Human Resources Specialist, Briana Geleta at 850-645-2731; the Associate Director of Environmental Health and Safety, Laymon Gray at 850-645-2279; and the Division of Workers Compensation Claims at 1-800-219-8953. Once this has been done, the accident should be reported to AmeriSys at 1-800-455-2079.

  • Q: How do I report a work-related accident?
    A: Immediately notify your supervisor/designated department representative. Accidents must be reported within 30 days of knowledge that the cause of your condition could be work-related.
  • Q: How is medical treatment provided for a work related injury?
    A: All medical treatment (except for emergency care) must be authorized by AmeriSys. After you notify your supervisor/designated department representative, they will report the accident to AmeriSys at 1-800-455-2079. AmeriSys will authorize medical treatment at the nearest medical facility. If you do not seek medical treatment from an authorized provider, you are responsible for paying for your medical expenses.
  • Q: Do I have to seek medical care immediately?
    A: No, but you must report your accident so that AmeriSys can create a record in case treatment is required at a later date.
  • Q: What happens if I am not able to return to work following an injury?
    A: Florida State University provides USPS, A&P, Executive Service, and salaried faculty employees up to forty hours of disability leave. The forty hours of disability leave can be used for authorized medical appointments or days off from work that has been certified by an authorized Workers' Compensation physician.
  • Q: What happens when the initial forty hours of disability benefits are exhausted?
    A: If you have returned to work and need to go back and forth to medical appointments, you must use your own leave time. Workers' Compensation benefits will begin once you have exhausted your 40 hours of disability leave and are placed out of work by your authorized Workers' Compensation physician.
  • Q: Who pays workers' compensation benefits?
    A: The Florida Department of Financial Services will assign a Claims Adjuster to your file. This person will be responsible for paying your workers' compensation benefits. Workers' Compensation Payments will be less than your full salary, but you may utilize your sick or personal leave to maintain your full-pay status.

Location Codes

When the supervisor/designated department representative contacts AmeriSys to report a Workers' Compensation claim, AmeriSys will ask for a 4-digit location code. AmeriSys will assign the location code to the Workers' Compensation claim for billing purposes. The supervisor/designated department representative will choose the location code based on the salary account name (Department Category) the injured worker is being paid a salary.
